Guest speakers, LAA members, and their spouses are requested to complete a Consent and Release of Rights Form. This form grants the LAA permission to use images, videos, audio, and other recordings for use in websites, newsletters, presentations, and other material promoting the activities of the LAA.

You can either provide a digital signature or draw an ink signature with a stylus or finger to sign the form.
